What is the diagnosis?
Nodular melanoma, vertical growth phase 1.2 mm Breslow depth.
This amelanotic melanoma is extremely hard to recognise clinically or by dermoscopy. It is symmetrical. There is no pigment network. The symmetrically arranged globules can be mistaken for the comedo-like openings of a seborrhoeic keratosis. However, the central amorphous area has irregular linear blood vessels, a subtle sign of malignancy.
The lesion was removed because the patient was worried about it – a new lesion that ‘felt funny’.
