The seemingly diverse conditions illustrated in this month’s quiz are cutaneous signs of metabolic syndrome.
Metabolic syndrome refers to the combination of abdominal obesity, hypertension, dyslipidaemia and insulin resistance.
For each of the twelve cases, study the image(s) and then answer the questions. You can click on the image to view a larger version if required.

Venous disease complicated by cellulitis
What is the cause?
Venous disease is also common in obese subjects due to obstruction to venous return. The image demonstrates the classical ‘champagne-bottle’ leg. Note hyperpigmentation, dermatosclerosis, dryness, varicosities of the right leg. The left leg is swollen and red with lymphoedema and cellulitis. Bacterial infection is a common complication of venous disease, but is particularly prevalent in obesity and type 2 diabetes.
