This patient has diabetes and renal disease. There is a 4-month history of these exuberant lesions on the back.
This is acquired reactive perforating collagenosis. Reactive perforating collagenosis is a rare papulonodular cutaneous disorder characterised by adherent keratotic plugs and crusts. It has strong associations with diabetes mellitus with chronic renal disease.
Histological analysis showed brightly eosinophilic collagen fibers being eliminated transepidermally with an overlying hyperkeratotic cap.
