A skin biopsy
Histology shows a proliferation of clear cells within the epidermis. What is the name for the skin disorder, which is more commonly seen on the nipple?
Extramammary Paget´s disease. This is a poorly understood intraepidermal neoplasm or unknown origin - probably a pluripotential epithelial cell that differentiates towards sweat gland tissue.
What is the appropriate management?
The affected area should be completely excised with a wide margin. Look for an associated adenocarcinoma; about 15% of cases of extramammary Paget´s disease arise in association with a local tumour of sweat gland origin or a distant carcinoma involving the breast or urogenital tract. Follow-up carefully; marginal recurrence is common and requires re-excision.
