What is the most likely cause?
Saprophytic mould
How would you treat it?
Saprophytic mould infection of toenails may be indistinguishable clinically from onychomycosis due to dermatophytes and may be more difficult to eradicate. Culture may reveal scopulariopsis, fusarium, aspergillus, alternaria, acremonium or scytalidinum.
Combined broad-spectrum topical amorolfine and oral terbinafine / itraconazole may improve but rarely cure nail infection due to moulds. Topical therapy may also be of benefit.
